Brief Summary in Lay Language

Demographic aging requires innovative approaches including individual support for older people, especially in their own homes. The NetzWerk GesundAktiv (NWGA) consisted of case and network management components and was implemented in the district of Hamburg-Eimsbüttel. The project aimed to enable participants to live independently and self-determined in their own homes for as long as possible. The target group were older people (70 years and older) with an increased risk of needing assistance and care (classification according to LUCAS Functional Index) or mild to moderate need for care (long-term care grade 1 to 3). The intervention consists of a geriatric-gerontological and social assessment, interdisciplinary case conferences, appropriate health promotion/prevention services and a case management. The care approach intended to ensure the preservation of independence and to prevent an onset or progression in long-term care grade. Within this study the intervention group was compared to a matched control group from similar urban areas. The observation period was 21 months.

Brief Summary in Scientific Language

Due to demographic changes, the elderly population in western countries is constantly growing. As the risk of functional decline and multimorbidity increases with age, health care systems need to face the challenge of high demand for health care services and related costs. Therefore, innovative health care approaches and geriatric screenings are needed to provide individualised care. This study aimed to expand the state of research by investigating the effectiveness of a multi-component care approach for the elderly in a German community setting.
A prospective, quasi-experimental study was initiated by statutory health insurance (SHI) companies (Techniker Krankenkasse, Barmer, DAK Gesundheit, Knappschaft). The innovative care approach included a geriatric assessment, a case and network management as well as digital supporting tools and was implemented at the Center for Geriatrics and Gerontology (Albertinen Haus, Hamburg-Eimsbüttel). Participants of the intervention were compared to matched controls recruited in comparable urban areas. The primary outcome measure was the progression in long-term care grade during the period of observation (21 months), which was analysed on the basis of SHI claims data.

  •   The intervention group passes a geriatric-geronotological/ social assessment. The results are discussed in interdisciplinary conferences which leads to an individual support plan. Participants recieve adequate offers (Social/ Health promotion/ Prevention) and are supported by case managers.
  •   Appropriate controls (usual care) were assigned to the intervention participants via exact matching (1:2).
