Brief Summary in Lay Language

After discharge from hospital, stroke survivors depent on continuity of care in the community, in order reduce functional impairments and the risk of recurrent events. In the community setting, patients face great challenges: the structure of the health care system is fragmented and sometimes confusing. The INDIKA project supports stroke survivors by providing a coordinated care. First, participants receive an individual case management by qualified nurses, second, they are integrated into a local health a provider network where health care providers from different professions collaborate.

Brief Summary in Scientific Language

The study evaluates the effectiveness of a complex intervention for stroke and TIA survivors in the community. This complex intervention consists of an individual case management and the integration into a local multiprofessional health care provider network. Participants are compared to patients receiving usual care regarding utilization of adequate health services, recurrent strokes, readmissions and mortality. Analysis is based on health insurance claims data from participants and usual care patients. This control group is constructed by exact matching using parameters such as age, sex, length of stay in hospital etc.

  •   Coordinated care through face-to-face case management for high-risk patients (serious functional impairments and care dependency): initial visit at home for assessing patient needs and developing an individual treatment plan; follow-up telephone calls every month and home visits every three months; support and coordination by the local health care provider network.
  •   Coordinated care through telephone-based case management: information and advice on stroke-specific health care and health care resources; if necessary support with appointments or applications; follow-up calls every three months; support and coordination by the local health care provider network.
