Brief Summary in Scientific Language

In times of demographic change, the number of patients suffering from one or several chronic diseases will increase further. These patients need complex health and social care.
The resulting transitions between different health and social care providers require comprehensive organization in order to achieve satisfying outcomes from medical, financial and patient perspectives.

For this purpose, a transformation towards a value-based, patient-centered care is necessary.

The aim of this project is to establish a network of health care research and practice for Cologne as a model region.
It is intended to promote exchange of ideas as well as new, innovative health care (research) projects. The emphasis is laid on organizational learning and the improvement of patient-centered, value-based care.



The network will be established by three projects (LYOL-C, MenDis-CHD and OrgValue). On the long term, further projects are supposed to be included in CoRe-Net. Those will gain the opportunity to benefit from the existing (data) infrastructure and thus to approach further issues of relevance to regional health care.

The network shares a common research infrastructure.
The three projects benefit from the establishment of a collective database which comprises SHI routine data, social and regional statistics as well as field data. Its analysis will be conducted goal-oriented.

The project LYOL-C (Last Year of Life Study in Cologne) examines the last year of life of patients deceased in Cologne.

The project MenDis-CHD (Quality of care regarding the detection and treatment of mental disorders in patients with coronary heart disease) focuses on the detection and treatment of comorbid mental disorders in patients with coronary heart disease.

Both projects examine cross-sectoral care trajectories and their patient-centeredness as well as patients’ and relatives’ preferences.
Furthermore, all care organizations involved are identified (e. g. hospitals, nursing homes, hospices, rehabilitation clinics, general and specialized practices).

OrgValue (Characteristics of Value-Based Health and Social Care from Organizations’ Perspective) connects the former mentioned projects LYOL-C and MenDis-CHD. It analyzes all organizations involved in the care of the above-mentioned groups of patients in order to define organizational characteristics that hinder or facilitate patient-centered care.
The findings help to develop and evaluate instruments for possible, necessary change processes.

Interventions/Observational Groups

  •   Patient survey applying the following questionnaires: PACIC, EQ-5D, HADS-D, SCID I, DemTect, SOEP, CERAD; additionally, self-developed items are used.
    Face-to-face interviews with triads of patients, relatives and health care providers. Interviews regarding care trajectories, perceived quality of care, attitudes, needs, preferences, barriers towards care.

    Survey of relatives applying the following questionnaires: VOICES, PACIC, SOEP, Items of QoDD-Ang; additionally self-developed items are used.
    Face-to-face interviews with triads of patients, relatives and health care providers. Interviews regarding care trajectories, perceived quality of care, attitudes, needs, preferences, barriers towards care.

    Survey of health care providers applying the following questionnaires: Decision-maker survey, SOEP, Structures and Environment (self-developed), HSCOs, ACIC, COSCO, Change Attitude Scale, Change Fatigue, HLHCO-10, Health Care Organizations’ Board Governance, TLI, Process-based hospital organization, Supply-chain-integration.

    Face-to-face interviews with triads of patients, relatives and health care providers, focus groups with patients, survey of particular health care providers included in LYOL-C and MenDis-CHD.
    Interviews regarding care trajectories, perceived quality of care, attitudes, needs, preferences, barriers towards care.

Additional Inclusion Criteria

MenDis-CHD:
1.) Adults with clinical manifest coronary heart disease (i. e. status post stable or unstable angina pectoris, acute coronary syndrome, after percutaneous coronary intervention and bypass surgery). Approx. 50% of the total patient population shall be female. Approx. 30% shall have a left ventricular ejection volume of ≤ 40% and thus have a cardiac insufficiency (Curtis et al., 2003).
2.) Relatives of these patients.

LYOL-C:
1.) The bereaved of patients who had been cared for within the region of Cologne and have deceased.
To the group of the bereaved, all relatives and close people are considered who cared for the respective patient and/or stood by his/her side during the last year of life.

OrgValue:
1.) Employees or decision-makers of a health care organization in Cologne.

All Studies:
1.) Subjects must be a German native-speaker or have adequate langue proficiency in German (incl. reading ability).
2.) Minimum age 18 years
3.) Written consent to the participation in the study
