Brief Summary in Lay Language

The aim of my doctoral thesis is to gain inside about the experiences of patients their caregivers with regard to patient care and to develop recommendations for the improvement of the processes in the context of follow-up care. In addition, the experience of clinical experts (physicians, therapists) is also going to be recorded.

Brief Summary in Scientific Language

Background: In Germany, a qualified and efficient post-stroke care after a medical rehabilitation has not been available so far, due to the strict separation of inpatient and outpatient treatments. The purpose of this study is to develop recommendations for the follow-upcare of stroke patients.
Stroke patients, relatives, neurologists, general practitioners and outpatient physiotherapists are to be consulted on the topic of follow-up care. Questioning will be done through paper-pencil and online- surveys and additionally with interviews.

No hypotheses are to be tested.

In the second step, stroke patients, caregivers, neurologists, general physicians and physiotherapists are interviewed in individual interviews regarding follow-up care

Primary Outcome

The goal of this study is to use a modified questionnaire to collect experiences and desires of stroke patients and relatives. Data collection is carried out for a period of 5 months. In addition, the clinical experts are interviewed with a short online survey. Data collection is carried out for a period of 9 weeks.

Stroke patients in phase C/D

Exclusion Criteria

Patients with severe motor and cognitive impairments.

Patients who cannot return to their previous home environment after completing the medical rehabilitation, due to serious impairments (for example, short-term care or a nursing home).
