Brief Summary in Lay Language
This project seeks to determine the role of approach and avoidancebehavior and related learning processes in chronic musculoskeletal pain. A special focus will be on differential roles of pain-related (pain relief) versus pain-unrelated reward during learning. We expect patients with chronic back pain to be characterized by reduced appetitive approach and enhanced avoidance as well as by reduced learning processes for reward and pain-relief. Additionally, learning-related processes in reward (pain relief) related brain regions is expected to be reduced in chronic back pain.

This project seeks to determine the role of approach and avoidance and related pavlovian and instrumental conditioning processes involving pavlovian-instrumental transfer in chronic musculoskeletal pain and will examine behavioral, peripheral and central correlates. A special focus will be on differential roles of pain-related (pain relief) versus pain-unrelated (monetary) reward during instrumental and pavlovian conditioning and their interaction. We will test a total of 50 back pain patients and 50 matched healthy controls in a pain-related approach-avoidance task and a pavlovian-instrumental transfer task. We expect patients with chronic back pain to be characterized by reduced appetitive approach and enhanced avoidance as well as by reduced pavlovian and instrumental reward- and pain-relief-related conditioning and reduced approach-related pavlovian-instrumental transfer. Additionally, learning-related aversive prediction error signaling in reward (pain relief) related brain regions is expected to be reduced in chronic back pain.

Primary Outcome
Questionnaires on pain behaviour, anxiet and depression and tasks (condiitioning) will be obtained once only, i.e. at one measuerment time point, for each participant (within the study period 01.01.2015 - 31.12.2016)

- Both, male and female

Additional Inclusion Criteria
Inclusion criteria for the CBP patients will be pain localized to the upper or lower back, minimum pain intensity 4 on a 0-10 point scale, minimum frequency 3 times/week, more than 6 months of pain.
Exclusion Criteria
Exclusion criteria will be chronic and current substance abuse, any neurological disease, left-handedness, major illness, pregnancy, a pacemaker or metal parts in the body, and for the CBP additionally pain after traumatic experiences/accidents, pain surgery, pain due to physical decline.
