Brief Summary in Lay Language
In Germany, the number of patients dying from chronic heart failure (CHF) is higher than the number of patients dying from cancer. Patients with CHF suffer from similar symptoms than patients with cancer (e.g. pain, fatigue, dyspnoe) and the symptom burden of both patient populations are comparable. Despite this, less patients with CHF than patients with cancer receive palliative care.
The present study aims (1) to investigate barriers and facilitators to palliative care perceived by physicians, cardiologists and nurses and (2) tries to develop recommendations to improve palliatice care of CHF patients.
Brief Summary in Scientific Language
Despite a high prevalence rate, similar symptoms and a comparable symptom burden less patients with chronic heart failure (CHF) receive palliative care than patients with cancer, even though some studies could proof that palliative care is effective inCHF patients. Studies from outside Germany revealed that palliative care of CHF patients is lacking because of the following reasons (1) lack of knowledge of health care providers (HCP) about what palliative care is, (2) not clearly defined responsibilities of HCP regarding palliative care, (3) lack of communication between HCP and (4) difficulties, to identify the right moment to refer CHF patients to palliative care. For Germany, no data about perceived barriers and facilitators of HCP to refer CHF patients to palliative care could be identified.
Therefore, the present study aims to:
(1) identify HCP attitudes towards palliative care of patients with CHF
(2) identify HCP perceived barriers and facilitators to palliative care of CHF patients
(3) develop recommendations based on HCP perceived barriers and facilitators to palliative care of CHF patients
Attitudes towards palliative care of patients with CHF and perceived barriers and facilitators to palliative care of CHF patients will be evaluated using a qualitative (interview) and quantitative (questionnaire) design. Recommendations will be developed using a delphi technique.

- Physicians and nurses will be asked about their attitude towards and experiences with palliative care of patients with heart failure. Delphi technique will be used to develop recommendations to foster palliative care in patients with heart failure.

- Physicians (cardiologist, general practitioner) and nurses (including heart failure nurses)
