Brief Summary in Lay Language

Background: For many physicians it is difficult to get a long-term transfer of what they have learned in a Communication Skills Training (CST) into their clinical practice. Therefor participants of a consensus meeting about CST demand to focus on the transfer of aquired skills into daily practise and the evaluation such trainings in real clinical situations. An effective possibility is to focus on individual learning goals and on individualized coaching, oriented on the conditions of the clinical everyday-life of each physician. Research question: The main goal of this project is to investigate, if and how much an intensive coaching improves the transfer of learned communication skills into the clinical practise. Methods: For this purpose the behavior of physicians working with cancer outpatients will be investigated. Before the workshop (t0) consultations will be videotaped (after consent) and questionnaires handed out. These consultations will be analyzed afterwards in the workshop and will build the basis for the formulation of individual learning goals; those will be used and applied in role play with actor patients. After the workshop more consultations will be videotaped; these will be used for coaching sessions. The intervention group (IG) will get four coaching sessions, the control group (CG) only one. Independent raters will use a checklist, which was developed for a pilot project, in order to evaluate the recorded consultations. It is expected that physicians, which got an intensive coaching can transfer better the learned communication skills into clinical practice.

A. Main research question:
Does intensive coaching improve the transfer of learned communication skills into clinical practise?
Hypothesis:
Physicians, which got five coaching sessions (IG), will show more and better communication skills than physicians, which got only one coaching session (CG) (Evaluation of videotaped consultations by blind raters)
B. Secondary research questions and hypothesis, which investigate the change on the competence feeling of the physicians and the evaluation of the consultations by the patients:
1 Does intensive coaching improve the feeling of competence of physicians?
Hypothesis: Physicians of IG will show a greater increase in the feeling of competence in regard to their learning goals than the physicians of the CG.
2 Does intensive coaching improve the evaluation of the consultation by the patients?
Hypothesis: Patients will evaluate the consultations with physicians of the IG in regard to their learning goals better than the consultations with physicians of the CG in relation to their learning goals.
C. Research question, which investigates the long-term effect of the coaching:
3 Does the effects of the modified training are preserved after three months?
Hypothesis: Physicians of the IG will be better evaluated than the physician of the CG three months after their last coaching session (a) by the raters, (b) by themselves (competence feeling) and (c) by the patients.
D. Explorative research question:
4 By means of qualitative interviews it will be investigated, which expectations patients have on the medical consultations and if the physicians of the IG fulfill the expectations of the patients better than the physicians of the CG.

Primary Outcome

Improvement of the communication skills, defined by means of criteria of the ComOn-Coaching Rating Scales. The consultations will be filmed at T0 (before the CST), T1 (immediately after the CST) and T2 (after last coaching); the films will be rated by blind raters and the assessments will be compared.

Secondary Outcome

Competence feeling of the physicians, satisfaction of the patients. Assessment by means of questionnaires (analogue to the ComOn-Coaching Rating Scales, Niglio de Figueiredo et al. 2017) at T0 (before the CST), T1 (immediately after the CST), T2 (after last coaching) and T3 (catamnesis, 3 months after T2); results of the assessments will be compared.
